Non-urgent advice: What you need to register

The practice currently asks for the national registration form and its adult or child new-patient form. It recommends evidence of identity and address to help match existing NHS records. NHS inform states that a person should not be delayed or refused registration only because they cannot provide identification or proof of address; contact the practice if this applies to you.

  • Your full name, date of birth and current address
  • Your previous GP practice, if known
  • Your CHI number, if known
  • Your current medicines, allergies and ongoing care
  • The appropriate practice new-patient form for each adult or child

After registration

Services are available after the practice has accepted and added the registration. Records may take time to arrive from the previous practice, particularly after a transfer from outside NHS Scotland. Ask for advice in good time if you take regular medicine.

Change your contact details

Tell the practice promptly if your name, address or telephone number changes so your record and invitations remain accurate.
